What is DPC?
Direct Primary Care is a 3-month minimum subscription service plan with Silver Mountain Pediatrics.
This plan is most ideal for families who opt to self-pay or for families whose insurance Silver Mountain Pediatrics cannot accept.

Direct Primary Care subscription plans are not insurance. Subscription plans do not provide unlimited care or access to pediatric nurse practitioners or other staff members. Silver Mountain Pediatrics does not offer mid-month refunds.

Must be a family who opts to self-pay or a family whose insurance SMP cannot accept
Up to date on Well Child Checks
Able to commit to at least a 6-month subscription plan
Automatic payment authorization

0 mos - 35 mos = $150 monthly
3 years (36 mos) - 6 years = $110 monthly
7 years - 12 years = $95 monthly
13 - 21 = $90 monthly
Silver Mountain Pediatrics will consider a Family Cap for families of 4 children at $350 monthly

Sick visits
Well Child Checks which will include standard American Academy of Pediatrics recommended screenings, in house point-of-care labs (lead, hemoglobin), vision and hearing
School documentation coordination
Vaccines will be provided through VFC (Vaccines for Children), as long as patients are uninsured
Access to education classes such as nutrition, sleep at no additional cost

Emergency care
After hours care. After hours care for ALL patients, will be directed to CHCO Nurse Triage line
Hospital care
Chronic/speciality care management
Labs and imaging will be referred out and families will be responsible for the bill
Point-of-care labs may be done in house, but for anything additional, patients will receive third party billing through chosen institution
